All applications must include a non-refundable fee to begin processing by the Board.

All NEW applications in North Carolina require an FBI Fingerprint Criminal Background Check. Information is sent to the applicant via email after the online application is completed and all required fees paid. The criminal background check can take up to 60 days after fingerprints are submitted, it is strongly recommended that applicants complete this part of the process as soon as possible after filing the online application and paying all required fees.

The application is active for one year from the date of submission after which it will be destroyed if it has not been completed, including the furnishing of all documents and records that are requested. File to be destroyed per the state functional record retention schedule. 

Note: Applicants without a social security number at the time of application should consider waiting to submit an application until after it has been received. A license cannot be granted without a social security number.

  LICENSURE BY EXAMINATION

For applicants who will take or have taken the National Physical Therapy Exam (NPTE) AND have never been licensed in another state.

  LICENSURE BY ENDORSEMENT

For applicants who have passed the National Physical Therapy Exam (NPTE) AND hold an active license in another U.S. state or jurisdiction.

NOTE: Military Member/Military Member Spouse with active military orders that brought them to the state of North Carolina should consult the Military Information page BEFORE beginning an application.

NOTE: License Revival is for previous North Carolina license holders who want to reinstate a lapsed license.
