All applications must include a non-refundable fee to be accepted. Note: The FBI Fingerprint Criminal Background Check may take up to 90 days.

The application is active for one year from the date of submission after which it will be destroyed if it has not been completed, including the furnishing of all documents and records that are requested. File to be destroyed per the state functional record retention schedule. 

Note: Applicants without a social security number at the time of application should consider waiting to submit an application until after it has been received. A license cannot be granted without a social security number.

  Exam Applicants

Exam Applicants have never passed the National Physical Therapy Examination (NPTE). See www.FSBPT.org for information on the NPTE and deadlines.

  Endorsement Applicants

Endorsement Applicants have passed the National Physical Therapy Examination (NPTE) and hold an active license in another state.

  Exam Out of State Applicants

Exam Out of State Applicants have or will take the National Physical Therapy Examination (NPTE) in a state other than North Carolina, but do not have an active license in another state.


 All new PTs and PTAs licensed in the current license year (February 1, 2023 – January 31, 2024) must renew by January 31, 2024, 5 pm EST.

NOTE: License Revival is for previous North Carolina license holders who want to reinstate a lapsed license.
