Please note: The NC Board of PT Examiners places the highest priority on protecting the safety and welfare of the citizens of North Carolina and therefor takes seriously its responsibility to ensure the minimum level of competence for the privilege of the license to practice physical therapy in the state. In that endeavor, there could be a longer than normal processing time of new applications for licensure. The Board regrets the inconvenience and is working diligently to move forward as expeditiously as possible.


New Applications

An initial application for licensure as a PT or PTA in the state of North Carolina.

All NEW applications in North Carolina require an FBI Criminal Background Check. Once the online application is completed and all required fees are paid, information will be sent to the applicant via email.
NOTE: Military Member/Military Member Spouse with active military orders that brought them to the state of North Carolina should consult the Military Information page before beginning an application.
License Revival

Previously licensed in North Carolina and want to reinstate a lapsed license.

Compact Privilege

A Compact Privilege enables a licensee to work in a Compact member state other than your home state.




The application is active for one year from the date of submission after which it will be destroyed if it has not been completed, including the furnishing of all documents and records that are requested. File to be destroyed per the state functional record retention schedule. 

Note: Applicants without a social security number at the time of application should consider waiting to submit an application until after it has been received. A license cannot be granted without a social security number.
