File a Complaint

Reporting Potential Violations of the Practice Act or Board Rules to the NC Board of Physical Therapy Examiners

The Board has the authority to investigate complaints regarding physical therapists or physical therapist assistants who violate the North Carolina Physical Therapy Practice Act or Board rules. The Board does not have the authority to resolve disputes over billing or to represent those persons who file complaints in actions against licensees. For the Board to properly address a potential violation, the following information must be provided in writing and sent to the Executive Director:

  • Name and address of licensee
  • Detailed statement of complaint
  • Names and contact information for people who can support the complaint.
  • Name and contact information of person filing the complaint.

For the Board to effectively investigate a complaint, the Board must be provided detailed information about the alleged violation and be provided with the name and contact information of the person filing the complaint. The Board is unable to investigate and prosecute anonymous complaints. In those instances where complaints are contested, it may be necessary for the person filing the complaint and/or witnesses to testify before the Board.
