File a Complaint

Reporting Potential Violations of the Practice Act or Board Rules to the NC Board of Physical Therapy Examiners

The Board has the authority to investigate complaints regarding physical therapists or physical therapist assistants who violate the North Carolina Physical Therapy Practice Act or the Board’s rules.  The Board does not have the authority to resolve disputes over charges or to represent complainants in actions against licensees.  For the Board to properly address a potential violation, the following information must be provided in writing and sent to the Executive Director:

  • Name and address of licensee
  • Detailed statement of complaint
  • Names and contact information for persons who can and will testify in support of the complaint.
  • Name and contact information of complainant.

Anonymous complaints often present difficulties during investigations because licensees are generally entitled to receive complete information regarding charges that are filed against them, and no disciplinary action can be taken without presenting evidence of violations against the licensee.  All efforts to maintain anonymity will be respected, but complaints can be dismissed if no other evidence against the licensee can be developed.
