Disclaimer for Disciplinary Action Summary

Position: Length of Time Disciplinary Actions Remain Posted on the Web Page - adopted March 09, 2000; revised August 24, 2017

When the Board takes disciplinary action against a licensee, a brief statement of the conduct and the sanctions imposed will appear on the web page for a period that coincides with the length of the disciplinary action, one year, whichever is greater, or the license has lapsed. Additionally, licensees with consent orders who fail to meet the conditions in their order may remain on the web page until all requirements are met.

The reports contained as part of this web site represent summaries of those formal disciplinary orders issued by the North Carolina Board of Physical Therapy Examiners imposing warnings, suspensions, revocations, and other discipline.

Disciplinary orders for individual licensees may be viewed under the Licensee Lookup tab on the home page. To access documents related to the disciplinary action:

  • Enter either name of licensee or license number and click Search button.
  • Click on the license number.
  • Click on the highlighted link in “Note/Disciplinary Actions/Restrictions” to view documents.

Current Disciplinary Actions

Cook, Andrew L., PT (Warning)
Location: Mooresville, NC, Iredell County

Conduct: Failure to apply for renewal and practicing with an expired license    
Discipline: Warning letter added to file; reimburse the Board for costs of the investigation. (Effective – March 7, 2024)

Emmerich, Christopher H., PT (Suspension)
Location: Kinston, NC, Lenoir County
License #: P13380
Conduct: Unable to practice safely by reason of Alcohol or other substance abuse   
Discipline: Suspension with conditions; reimburse the Board for costs of the investigation. (Effective – June 16, 2022)

Hurley, Brandy Michelle, PTA (Suspension) 
Location: Wadesboro, NC, Anson County

Conduct: Removing prescription medications from a patient’s home
Discipline: License surrender 6 months; Inactive suspension 18 months.  (Effective Date – December 14, 2017)

Scott, Julian W., PT (Suspension)

Conduct: Recording false or misleading data, measurements or notes regarding a patient; willfully or intentionally communicating false or misleading information regarding a patient; and engaging in a misrepresentation or deceit, or exercising undue influence over a patient or former patient for the financial gain of the patient.

N.C.G.S. §90-270.102 (3) Practice or attempt to practice physical therapy with a revoked, lapsed, or suspended license.

21 NCAC 48G .0601 (13) altering a license or renewal card by changing any information appearing thereon; (14) using a license or renewal card that has been altered; (17) violating any term of probation, condition, or limitation imposed on the licensee by the Board.

NCGS 90-270.103 (6) Engaging in any act or practice violative of any of the provisions of this Article or of any of the rules and regulations adopted by the Board, or aiding, abetting or assisting any other person in the violation of the same; (7) The commission of an act or acts of malpractice, gross negligence or incompetence in the practice of physical therapy; (8) Practice as a licensed physical therapist or physical therapist assistant without a valid certificate of renewal; (9) Engaging in conduct that could result in harm or injury to the public.

Discipline: Suspension – 6 months; followed by conditions for 18 months and, reimburse the Board for costs of the investigation. (Effective – October 30, 2022)

Suspension – 36 months; commencing on the effective date of the 2022 Order (October 23, 2022) with conditions and, reimburse the Board for costs of the investigation. (Effective – June 13, 2023)

Steinbach, David, PT (Suspension)
Location: Cornelius, NC, Mecklenburg County
License #: P19816
Conduct: Commission of an act or acts of malpractice, gross negligence or incompetence in the practice of physical therapy; and engaging in conduct that could result in harm or injury to the public.    
Discipline: Suspension with conditions; reimburse the Board for costs of the investigation. (Effective – June 22, 2022)
