Additional requirements needed to complete a licensure application are located on the last page of the online application and accessible after all the online sections have been completed.

As documents are received and processed, the application status page will be updated with a date indicating receipt. Applicants will be notified of any issues with a document.

  • CHARACTER REFERENCES - Two (2) are required from someone who has known the applicant for at least one year and is not a relative or spouse. After information is provided by the applicant, references are sent an email containing a secure link to complete the reference.
  • OFFICIAL TRANSCRIPT / Education Certification  - Must be sent directly (electronically or by mail) from the educational institution to NCBPTE and include both degree and date of graduation. For foreign-trained (non-CAPTE accredited) and military-trained applicants, credentials must be evaluated by a FSBPT accepted credentialing service.   • NPTE SCORES - If registered for the exam through North Carolina, scores are automatically sent directly to NCBPTE. Request a score transfer from FSBPT (www.fsbpt.org), if your exam jurisdiction was other than North Carolina. Contact the Board for questions about scores not housed at FSBPT.
  • FBI FINGERPRINT CRIMINAL BACKGROUND CHECK - Detailed Information is sent by email and the required forms become accessible on the website after the online application is completed.
    NOTE: Once the fingerprints are submitted, it can take up to 60 days before the results are received by the Board office. If fingerprints are mailed to NCBPTE an Authority for Release of Information (ARI) form is required to be on file and can be emailed or uploaded. If fingerprints are done through livescan an Electronic Release of Information (ERI) is required to be on file and can be emailed or uploaded.
  • JURISPRUDENCE EXERCISE - All new applicants (examination and endorsement) are required to complete Jurisprudence Exercise 1 before the application is sent to the Board for final approval. This exercise is available on the Board website to be completed online, and there is no fee. A report is automatically generated for the Board to review.
  • PHOTO OR MILITARY ID - Upload items while completing the online application or at a later date using the upload feature on the applicant dashboard.
  • LICENSE VERIFICATION - Endorsement applicants must have an active license verification sent directly (mail or electronically) from one United States Physical Therapy Licensing Board to NCBPTE. For states that do not send verifications, contact the Board to confirm how NCBPTE will obtain the required documentation. Office staff wil verify licenses issued by the following states: AK, CA, CO, DE, FL, ID, MA, ME, MO, OR, RI, TN, TX, VA, WA, and DC.
