It is your responsibility to notify the board in 30 days (Board rule: 21 NCAC 48F. 0105) of any changes in
relevant contact information (name, home and/or business address, email, phone numbers) to ensure timely
receipt of important correspondence related to licensure and physical therapy regulation in North
Carolina.
Name Change
- Name changes require legal documentation showing the name change.
- Send an email to PTBOARD@ncptboard.org with "Name Change"
in the subject line.
- Include your full name as it currently appears on your license, your license number, your new name, and
date of birth.
- Attach ONE of the following supporting documents as a PDF:
- 1. Copy of a state-issued marriage license that includes the original signature and seal from
the
clerk
of court.
- 2. Copy of an official divorce decree restoring maiden name.
- 3. Copy of a court order showing the name change (adoption, legal name change, federal identity
change).
- A new wall certificate can be ordered from your personal dashboard. Cost is $30 paid online.
- A new wallet card can be printed from your personal dashaboard.
- If you are an Exam applicant taking the NPTE, your ID will need to match the information supplied to the
testing agency (FSBPT) or you may be denied access to the testing center. Check FSBPT.org for additional
details.
Email/Address Change
- If you are an applicant for licensure, we will mail the licensure packet to the address on file when
you
are licensed.
